As a Cytotechnologist or Cyto-Pathologist, atrophic cases, primarily due to natural degeneration of the parabasal cells in a background of fibrin, are big challenges and brainteasers while you screen for Cervical Cancer.
Degenerated parabasal cells, cellular debris, old blood and pollution of the samples by lubricants/ultrasound gels can lead to overdiagnosing as degenerated and/or bad stained parabasal cells can mimic HSIL / carcinoma cells. A smaller number of cells and stripped nuclei are further difficulty in analyzing atrophic samples.
Furthermore, carcinoma cells may also be misdiagnosed as normal atrophic cells!
Lubricants and ultrasound gels, which can affect staining quality and cell quantity on slides, should be avoided prior to sampling, a fact of which gynecologists must be aware.
